What is Management of Acute and Chronic Gallbladder Disease?

Management of acute and chronic gallbladder disease refers to a range of strategies employed in the treatment of acute (sudden and severe) and chronic (recurrent or long-standing) gallbladder conditions.

What is a Gallbladder?

The gallbladder is a pear-shaped organ located on the right side of the abdomen, just below the liver. It stores bile fluid, which is produced in the liver. Bile fluid contains water, proteins, fats, cholesterol, bile salts, and bile pigments, which aid in the digestion of food. The gall bladder releases bile fluid into the intestine through the common bile duct following the ingestion of food.

What is Gallbladder Disease?

Gallbladder disease is an umbrella term for several conditions that may affect the gallbladder, including infection, inflammation, or blockages. Cholelithiasis (gallstones) and cholecystitis (inflammation of the gallbladder) are the most common gallbladder diseases.

What are the Types of Gallbladder Diseases?

Common types of gallbladder diseases include the following:

Cholelithiasis

Cholelithiasis is the medical term for the presence of gallstones in the gallbladder. Gallstones are hardened deposits of digestive fluid, primarily cholesterol or bilirubin, that can vary in size. While many people with cholelithiasis are asymptomatic, some may develop biliary colic - a sudden, sharp pain in the right upper abdomen - especially after fatty meals.

Cholecystitis

Cholecystitis is the inflammation of the gallbladder, often caused by a gallstone blocking the cystic duct. It can lead to sudden, severe pain in the upper right abdomen, fever, nausea, and vomiting. If untreated, it may result in serious complications like infection or rupture.

What are the Treatment Strategies Employed for Management of Acute and Chronic Gallbladder Disease?

The treatment strategies for managing acute and chronic gallbladder disease vary depending on the severity, symptoms, and underlying cause of the condition.

Acute Gallbladder Disease Management

This usually refers to acute cholecystitis, often caused by gallstones blocking the cystic duct. The primary management approach includes hospitalization, bowel rest (NPO), intravenous fluids, antibiotics to combat infection, and pain management. Early laparoscopic cholecystectomy - typically within 72 hours - is considered the standard treatment to remove the inflamed gallbladder and prevent recurrence. For patients who are not suitable candidates for immediate surgery due to other medical conditions, a percutaneous cholecystostomy (drainage tube insertion) may be performed as a temporary measure.

Chronic Gallbladder Disease Management

This often refers to chronic cholecystitis, caused by repeated episodes of inflammation due to gallstones. This is primarily managed through elective laparoscopic cholecystectomy, especially when patients experience recurrent biliary colic or complications like gallstone pancreatitis. In cases where surgery is not feasible, medical management includes dietary modifications (such as a low-fat diet), pain control with antispasmodics or NSAIDs, and occasionally bile acid therapy like ursodeoxycholic acid to dissolve cholesterol stones. Surveillance may be considered for asymptomatic gallstones unless high-risk features are present.

Overall, the goal is to relieve symptoms, prevent complications, and improve quality of life.
