What to Bring to Your Appointment
- Photo ID – Driver’s license or another valid government-issued ID.
- Insurance card(s) – Including primary and any secondary coverage.
- Referral letter – Required if your insurance plan needs one.
- Test results – Any relevant imaging (X-rays, MRIs, CTs), endoscopy reports, or labs. Bring printed copies or CDs.
- Medication list – Include names, dosages, and how often you take them.
